The new "On The Road" page allows you to add your assignment just like the old one, however, now the assignment is also displayed on a map.

The new form also contains a geo lookup and a map. (see the image below)

The form contains the following fields.

Your Name - This is your name and is a required field.

Which Vendor - Enter the name of the vendor you are working for.

Assignment Start Date -  Enter the date you started the assignment, you can click the calendar icon to select the date.

What Type of Claims:  Select the type of claims you are handling from the drop-down list.  If working more than one type, pick the type that you have the most of.

Street:  This is used for the Get Coordinates button, if you enter the street, city and state and click Get Coordinates it will try to locate the map coordinates.  However, you can also locate your location on the map and click it to add the coordinates.  Please street address is not shown anywhere but someone could zoom in and locate the street. We suggest if you use the Street that you use the office location or some other location and not your exact location. City and State is all that is displayed.

City: Enter the main city of your assignment location.

State: Enter the state of your assignment.

The Lat and Long coordinates are required and are added when you either click the button "Get Coordinates"  or by locating your location on the map and clicking the location.  You may need to use the zoom keys to locate the city, the closer you zoom more detail is shown on the map.
