Adjuster Preliminary Damage Assessment form (APDA)

The Federal Emergency Management Agency (FEMA) requires all NFIP Certified Adjusters, who adjust flood losses within the NFIP, adhere to the following:

· The Adjuster Preliminary Damage Assessment form (APDA) must be completed on buildings which sustain damages beyond 50% of the Replacement Cost Value (RCV).

o The APDA form may be downloaded at http://www.fema.gov/library/viewRecord.do?id=2580
o The APDA form must be faxed to (301) 577-3421 or e-mailed to iService_claims@ostglobal.com within 48hrs of the completed loss adjustment. Please indicate “APDA Enclosed” in the subject line of the e-mail.
